I think that this is an important post from No Shelf Required blog:
Accessibility and eBooks ? Resources and an Interview
The post offers recommended resources on eBook accessibility from Ken Petri, The Ohio State University Web Accessibility Center about legal issues, format accessibility, readers, and Organizations and Repositories.
Another useful source is the Inclusive Design Resource Centre at OCAD University in Toronto (formerly the Adaptive Technologies Resource Centre associated with the University of Toronto) as well as the CNIB, Canadian National Institute for the Blind.
Of course, every library should have someone designated to be aware of these issues and strive to improve access for all.
